I do take request and if I agree with them I will implement them.
In this case, in my opinion, it's important the image matches the action.
Meaning, if the image is an otk position, the instruction to do so will also be over the knee.
The same goes for the implement, you are always told to get the same implement as shown in the image by your disciplinarian.
Adding other locations to smack will be severe, I personally will really hate that!
I'm still also playing the script myself once in a while with my favorite disciplinarians.
But it is another good suggestion and I will see what I will do.
I need to give it some more thinking and some time but I will come with an update regarding your requests.

I still use the clinic quite often. It's a great script! Thanks for all the work you've put into it.
A couple of suggestions I'd like to throw out.

-Different positions during corner times. Kneeling, hands on head, hands folded behind back etc..
-Having to go perform chores in between rounds when being disciplined for ignoring or forgetting household chores.
-Integrating something like the motion detector in virtual corner time to make sure of not moving.
-The possibility of figging during corner time.
-Sometimes having to count between spanks "one thank you ma'm I will behave" etc..

Just my thoughts on things I would like to see.

Just to let you all know I'm working on the latest requests, I've updated the script with a new version.

As requested by cadsin, different spank positions are implemented for all disciplinarians now.
Check behind the button set - personal record and you can set your preference for this.

Not being an programmer, it's time consuming to implement and to get it to work even if it sometimes seems an easy request.
I will come with another new update for the other requests but that will take about another one or two weeks or so for me to get it done.

In the mean time.... enjoy, or not.

A few other suggestions/requests
would it be possible to add a punishment for driving while texting? Bad habit i'm trying to stop. Maybe too specific but I thought I'd ask.

Thoughts on maybe additional punishments in some scenarios
if being punished for drinking too much disciplinarian could forbid drinking alcohol for a period of time.
if being punished for eating unhealthy then some sort of diet restriction for a number of days only vegetarian or something like that.
if being punished for laziness maybe have to run the exercise tease scrip in between rounds.

Just some ideas might be a bit much but i figured id throw them out there.

A new update, letting you know I'm still working on it, although it goes very slowly.
In this update 3.6, as requested, directed spank locations. Will be continued.

Just a couple of things I noticed.

Mouth soaping seems to be triggering much more often than it should. Typically after every set.

When changing the option for spank locations, there's an invalid file error for /Images/DisciplineClinic/Anatomy . I don't think it's included in the discipline pack.

The anatomy.png file is included in the last upload and should be placed in images/DisciplinePack.

I've nothing changed regarding the mouth soaping so that's weird.
Keep in mind that the mouth soaping is dependent of the offense and the disciplinarian selected.
I'll see if I can find something wrong to improve.
